RELEASE and AUTHORIZATION:
The undersigned requests that The Small Business Link, LLC. and any of its affiliates including,
without limitation, their agents, and assigns (“THE LINK”), review Applicant’s credit for
commercial financing, which financing is to be used for business purposes only and not for
personal, family or household use (the “Financing”). The undersigned has advised THE LINK that
he or she is an owner, principal, proprietor, partner, member, officer, or an individual acting
as a guarantor of Applicant’s financial obligations with authority to act on behalf of
Applicant. The undersigned understands and acknowledges that his or her personal credit
worthiness, credit standing, credit capacity, character, general reputation, personal
characteristics, and/or mode of living may be a factor in the evaluation of the credit
worthiness of Applicant. Accordingly, the undersigned authorizes THE LINK to obtain and review
credit reports and/or conduct background investigations on any of the undersigned and/or
Applicant as may be necessary in the sole discretion of THE LINK to evaluate the Financing, any
future financings of or credit extensions to Applicant, or any matter arising in connection
thereto. The undersigned further understands and acknowledges that, (a) THE LINK may obtain such
reports and information from national credit bureaus, reporting agencies, trade organizations,
banks, finance companies or bonding companies and other third parties, and (b) THE LINK may
disclose to third parties certain financial information regarding Applicant that THE LINK has
obtained or will obtain in connection with the Financing, including without limitation, payment
history information, account balances, due dates and other payment information. The undersigned
hereby authorizes the release to THE LINK of the above information. Also, to help the government
fight the funding of terrorism and money laundering activities, federal law requires all
financial institutions to obtain, verify, and record information that identifies each person who
opens an account. What this means for you: When you open an account, we will ask for your name,
address, date of birth and other information that will allow us to identify you. We may also ask
to see your driver’s license or other identifying documents. A copy of this signed document
shall have the same validity as the original. The undersigned confirms the foregoing
acknowledgments and authorizations and ratifies any prior actions of THE LINK in connection with
any of the foregoing by executing where indicated below.

statement, overvalues any land, or property security, for the purpose of influencing THE LINK in
connection with a loan, is subject to a fine of up to $5,000, or imprisonment for up to two
years, or both. Under penalty of perjury, you certify that (i) all information given on this
application is true, complete, and is given for the purpose of requesting a loan; and (ii) that,
other than those you have stated on this application, you have no other outstanding
indebtedness, either as a maker, co-maker, or guarantor. You authorize any person, association,
or corporation to furnish on request THE LINK, information concerning you or your affairs. You
authorize THE LINK to contact and inquire about your references and your employer(s) present,
past and future; and to obtain consumer credit reports about you. You also authorize THE LINK to
furnish information concerning your account to credit reporting agencies. THE LINK may report
information about your account(s) to credit reporting agencies/bureaus. Late payments, missed
payments, insufficient funds transactions, or other defaults on your account may be reflected in
your credit report. You understand that you will be provided with a copy of the Agreements and
Disclosures applicable to your Invoice Factoring Loan Plan, including the Invoice Factoring Loan
Agreement and the Truth-in-Lending Disclosure, the Statement of Billing Rights, and the Security
Agreement, when the loan sub-account is approved. You further agree that if you sign this
Application and you access the line of credit, negotiate any check or draft that is issued under
the terms of the Loan Plan, or utilize the loan proceeds in any way, then you are agreeing to be
bound by the terms and conditions applicable to the Loan Plan, including the Promise to Pay all
amounts owed to THE LINK. If you are applying for closed-end credit, you acknowledge that you
will sign your Note and Disclosure Statement at the time the loan closes and you agree to be
bound by its terms and conditions. If this application is approved and invoice factoring loan
issued, the undersigned Applicant(s) by signing and using invoice factoring loan agree(s) that
the Applicant(s) will be bound by the terms and conditions of the invoice factoring loan account
agreement and disclosure and all amendments thereto.
